Once they hit, any further attacks are not against a flat-footed target unless using invisibility or something. That's how I play it. Seems lame to make a caster who successfully uses Stealth not to benefit from doing so. WebNow, it may ruin your stealth. The verbal component of spellcasting states that it must be done with specific pitch and resonance. You cannot achieve resonance with a whisper, so most DMs will say that spells must be cast with at least a normal speaking voice. That may ruin your "stealth".
